ABSTRACT

Population health is integral to achieving the triple aim in health care. Pharmacists are medication experts that can be utilized in the primary care setting to assist with population health, value-based initiatives. This session will describe the emergence of the term population health management and outline successful pharmacist-led population health management interventions implemented into primary care practice. Lessons learned from more than 5 years of population health management work will be summarized, allowing participants to have an up-close view of ways to be more efficient in this important area of work.
