ABSTRACT

PCMH incorporates the patient as a whole, not just the one issue that may bring them to the health care office on any given day. Addressing characteristics of medical indigence is critical for health care providers of vulnerable populations to reduce barriers and provide culturally and linguistically appropriate care. This presentation will focus on vulnerable populations with significant challenges regarding access to care. Best practices and exemplars from a highly successful and recognized primary care practice will be presented.
