Using the PCMH Model to Align Value-Based Incentives in a Network of Primary Care Clinics

ABSTRACT

As payers move toward value-based models, it is important to align priorities and focus clinical quality teams on items that add value to our contracts. Multi-practice primary care organizations have been challenged to develop Quality Improvement programs that align multiple payers needs and NCQA PCMH standards, while also maximizing potential incentives for value-based care. Application of PCMH foundational concepts to the process for identifying and prioritizing clinic-based interventions has helped align work across multiple clinics. Additionally, multi-practice systems can be challenged to integrate their Quality Improvement programs with a sustainable PCMH learning system.
