ABSTRACT

Moving the measure on diabetic HbA1C control can be a daunting task. Patients who are non-adherent likely remain non-adherent until they reach an “aha” moment. Visual management of blood sugar levels can be a telling story and an eye-opening experience for patients, as we have discovered with implementing continuous glucose monitoring (CGM) in our practice. Join us as we demonstrate patient improvements in HbA1C reduction, compare CGM vs. non-CGM patients, and most importantly work through barriers to improvement from social determinants of health.
