Community Health Workers: A Transformative Interface to the PCMH

ABSTRACT

Community Health Workers (CHWs) are professionals that support individual patients as they access and navigate our complex health system. They play an important role in ensuring that barriers to health care are identified, are resolved, and that the patient’s health improves. As health care moves away from a fee-for-services model to more value-based models of care, it is essential that a primary care practice meet the needs of their patient population even if it means addressing the non-clinical barriers to care that happens outside of their practice walls. Community Health Workers are specifically trained to assist is this area. This session is designed to educate participants on the value of utilizing CHWs in the practice setting and how the practice can yield a financial return on their investment as well as improvement in quality and patient satisfaction.
