- Outline patient- and provider-related barriers that contribute to suboptimal and costly type 2 diabetes care
- Implement strategies to overcome patient- and provider-related barriers to type 2 diabetes care, previously perceived outside of managed care’s scope
- Apply recent evidence on current and emerging type 2 diabetes pharmacotherapies to formulary discussions and review
- Develop or augment a type 2 diabetes disease management program, based on available resources, such as the NCQA Diabetes Recognition Program, current guidelines, and Comprehensive Diabetes Care HEDIS performance measure
